- Credit or Debit Card Requirement: A valid credit or debit card must be kept on file for all memberships.
- Lapse Policy: If a membership lapses for over 30 days, the monthly plan will require an activation fee to reinstate.
- Eligibility: This plan is designed for patients without dental insurance and cannot be combined with other offers or discounts.
- Benefit Duration: Benefits are available immediately upon enrollment and remain active for one year from the enrollment date.
- Purpose: This membership plan is not dental insurance but a tool to help patients and families access quality dental care at a reduced cost.
- Membership Card: No membership cards are issued; all membership information is securely stored at Miranda Dental.
- Refund Policy: Refunds are not available after 90 calendar days from enrollment.
